Veterans in the mountain communities can receive high-quality outpatient care close to home through the VA Community Care Network (CCN), the VA’s direct connection with community providers.
As an approved provider, Mountains Community Hospital offers essential outpatient services such as lab work, radiology, respiratory (EKGs) and physical therapy – all covered by VA benefits with proper authorization.
Instead of having to make the long drive down the hill, eligible veterans can choose MCH for their care. The VA determines eligibility based on six criteria, which are listed on the Veteran Community Care Eligibility Fact Sheet on VA.gov. The criteria include average drive time and appointment wait times – factors that often apply to mountain residents.
Follow these steps to receive care at MCH through the VA CCN:
- Confirm eligibility: Call (909) 825-7084 (press 6, then press 1) to check if you meet the VA’s requirements.
- Visit your VA primary care provider: If outpatient services are needed, you can request to have them done at Mountains Community Hospital.
- Referral process: Your VA primary care physician will send the referral to the VA’s CCN authorization department.
- Verify your request: A day or two after your visit, call CCN at (909) 307-5248 to confirm they received the request and specify “Mountains Community Hospital” as your preferred provider.
- Approval and scheduling: Once approved, both you and MCH will be notified, and you can schedule your appointment.
